The head term is the tip. Under “primary care doctor Tampa” sit “family doctor Tampa”, “doctor accepting new patients” plus a neighborhood, “same week appointment”, and every condition you treat with a town attached. “Family doctor Denver” alone is typed about 210 times a month. A practice's service lines and neighborhoods add up to dozens of searches like those, and holding them is the program's job.

It is also what a comparison shopper reads late at night.

When the Health System Outranks You on the Specialty Term, Win Everything Around It
Search “orthopedic surgeon” or “dermatologist” plus your city and the top organic results are almost always the health system, then Healthgrades and Zocdoc. The system has hundreds of physician profile pages, a long-trusted domain, and an SEO team. An independent practice does not beat that on the bare specialty term, and it does not have to.
Google ranks pages, not practices. The system's orthopedics page is one general page covering dozens of surgeons. Your page on rotator cuff repair recovery, or plantar fasciitis treatment in your part of town, is specific, and the specific page wins the condition search. The same goes for access terms the system rarely writes: same-week appointments, accepting new patients, a cash price for a visit.
Then there is the map pack. Google ranks those three listings on relevance, distance, and prominence, and distance is the one the health system cannot buy. A patient two miles from your office sees you before the hospital campus across town, if your profile is built out and your reviews are recent.
